How many times have we heard some version of this story?
“Hey doc, my tooth hurts every time I drink cold since you replaced my silver filling with that new white filling.”
Pulpal diagnosis is an extremely important and underused tool in restorative dentistry. Before moving forward on any major restorative work, this diagnosis should be completed. Endo-Ice is a critical tool that allows us to determine the vitality of the tooth. This will lead us to the pulpal diagnosis.
While that’s helpful, my favorite use for Endo-Ice is during local anesthesia. For example, after performing my mandibular block via a gow-gates and IAN, I wait for lip and tongue signs. When patients say they definitely have both those signs, I use Endo-Ice to check for any thermal sensitivity. If there is, I will not reinject for the mandibular block but rather go to my supplemental injections.
First, I start with an intraligamentary injection. If that doesn’t work I go to an intraosseous injection. Lastly, if all else fails, I use the intrapulpal injection.
